Executive Summary

Relational Infrastructure for Community Care

Cherub Care is a nonprofit connector that links individuals and families of limited resources to the services that can resolve their needs — and stays with them until the need is met. It is relational infrastructure for community care.

This is not a perpetual charity dependent on the next grant — it is an organization deliberately transitioning from philanthropic seed to earned revenue from the institutions that benefit when needs are resolved.

The Problem

Disconnection, Not Scarcity

<50%

Eligible families in aggregate receive less than half the value of the benefits they qualify for.

$Billions

In funded help go unclaimed each year — not for lack of resources, but for lack of connection.

The failure is not scarcity but disconnection. The most capable prior attempt, Benefits Data Trust, closed in 2024 — from finances it could not sustain, not from a flawed mission.

Cherub Care is designed around that lesson.

Our Design

The Model

A Connector, Not a Provider

We do not own the help. We assemble and coordinate it across government, nonprofit, and private resources.

Seven Need Areas

Food, health, housing, income, legal, work, and transportation — the full spectrum of what resolves a household's condition.

A Trusted-Voucher Network

A trusted word speeds the way but never guards the door. Authenticators open access — they do not close it to the unconnected.

Coordination Intelligence

Systems that match need to ready help — accounting for resource fit, availability, and likelihood of resolution.

Resolution, Not Referral

We stay until the need is met. We measure the outcome — the benefit received, the appointment kept, the protection in place.

Sustainability Strategy

The Earned-Revenue Model

Community care hub. Payer contracts for verified resolution. Four revenue streams built on mechanisms that already exist — not speculative policy change.

Payer / Source Mechanism
Medicaid MCOs & Health Plans Per-member or per-resolution contracts for social-need navigation
Health Systems & FQHCs Navigation tied to value-based contracts and community benefit obligations
States & Managed Programs Enrollment-and-retention services within existing state program structures
Community Giving The charitable ground that seeds trust and sustains the mission as earned revenue scales
Policy Anchor — Florida

Florida's Statewide Medicaid Managed Care (SMMC) program (2025–2030) emphasizes value-based payment and in-lieu-of-services authority. Revenue is anchored in mechanisms that already exist.
